Superb Shifts "Thankful for Your Team" Giveaway — Official Rules

NO PURCHASE OR PAYMENT NECESSARY TO ENTER OR WIN. A purchase or payment of any kind will not increase your chances of winning. Void where prohibited by law.

1. Sponsor. Superb Shifts Inc., 139 S 144th Street, Box 552, Omaha, NE 68154 ("Superb").

2. Eligibility. The Thankful for Your Team Giveaway (the "Giveaway") is open to healthcare facilities located in the United States that have an active Superb Shifts facility account in good standing on the drawing date (each, a "Facility"). Facilities whose accounts are managed entirely by Superb staff, with no facility users of their own, are not eligible. Entries are earned by and awarded to the Facility, not to individual users. Superb employees, staffing agencies and subcontractors, and their immediate family and household members are not eligible to receive the prize on a Facility's behalf. Each Facility is responsible for making sure that accepting the prize complies with its own gift and ethics policies and any laws that apply to it. A Facility that cannot accept the prize may decline it, and an alternate may be drawn.

3. Entry period. The Giveaway begins November 1, 2026 at 12:00 a.m. Central Time and ends November 30, 2026 at 11:59 p.m. Central Time (the "Entry Period"). Superb's records are the official timekeeping device for the Giveaway.

4. How to enter. There is nothing to sign up for. Entries are recorded automatically:

• 1 entry for each shift a Facility posts on Superb Shifts during the Entry Period, for any date, whether or not the shift is filled. This includes shifts Superb posts on the Facility's behalf.

• A shift the Facility later cancels or deletes does not earn an entry.

• A shift that needs more than one worker earns 1 entry for each opening.

5. Free entry. Each eligible Facility receives 1 free entry if at least one of its users logs in to Superb Shifts during the Entry Period, whether or not the Facility posts a shift. No other action is required, and the free entry is limited to one per Facility. Free entries have the same chance of winning as all other entries.

6. Drawing and odds. On or about December 1, 2026, Superb will randomly select one winning Facility from all eligible entries. Each entry is one chance to win. The odds of winning depend on the total number of eligible entries received.

7. Prize. One (1) prize: a $200 Visa gift card, intended for the winning Facility to treat its team to a meal. Approximate retail value: $200. The gift card is subject to the issuer's terms and conditions. The prize cannot be exchanged for cash or transferred to another facility. Superb may substitute a prize of equal or greater value if the stated prize becomes unavailable. Limit one prize per Facility.

8. Winner notification. Superb will notify the winning Facility by email, sent to the Facility's primary account contact, within 3 business days of the drawing. If the Facility does not respond within 7 days, it may forfeit the prize, and Superb may select an alternate winner.

9. Verification. Superb may verify all entries against its records. Superb may disqualify entries for shifts that were not posted in good faith, including shifts posted and cancelled to collect entries, or any other attempt to manipulate the Giveaway.

10. Publicity. With the winning Facility's permission, Superb may share the Facility's name and a photo or quote in Superb communications.

11. Taxes. The winning Facility is responsible for any taxes that may apply to the prize.

12. General conditions. By taking part, each Facility agrees to these Official Rules and to Superb's decisions, which are final. Superb is not responsible for technical failures, lost or incomplete data, or errors in recording entries. Superb may cancel, suspend or modify the Giveaway if it cannot run as planned. These Official Rules are governed by the laws of the State of Nebraska.

13. Privacy. Information collected in connection with the Giveaway is used to run the Giveaway and is handled under Superb's privacy policy at superbshifts.com/privacy-policy.

14. Winner's name. To request the name of the winning Facility, email info@superbshifts.com by December 31, 2026.
